We offer many types of care.

What we do:

Private Duty Care is our main service. It is the care we provide at your residence.

We also provide Geriatric Care Management, where we coordinate the care provided by others. Together, the two types of service entail these duties:

  • Manage care on behalf of relatives and trustees
  • Light housekeeping, meal preparation, laundry
  • Help with personal care, bathing, and dressing
  • Ambulatory assistance
  • Provide caring companionship
  • Driving, shopping, and outdoor activities
  • Keep track of medications and doctor’ visits
  • Assist with phone calls and correspondence

Geriatric Care Management

Older people often rely on family and friends to supervise their care. If your family and friends live far away, you can rely on us instead.

A trained professional from our office will assess your care needs, arrange services (our own and third-party), and monitor care on an ongoing basis.

  • Initial assessment and in-home evaluation of total care needs by RN
  • Personalized plan of care developed by RN
  • Recommendations/ Referrals for third-party services
  • Recommendations/ Referrals for home safety
  • Monthly nurse visit / update sent to family
  • Medication Management/ RN oversight
  • Nutritional needs and weight monitoring
  • Responsible to arrange or provide transportation to Doctor visits, labs, tests
  • RN Liaison between doctor and family

Medicaid Certified Care

We provide all types of care, including the care of children. This service must be authorized by, and is billed to, Medicaid.

Private Duty Care

Although we have referred to Private Duty Services as at-home-care, it doesn't have to be at home. We can serve in an assisted living facility or even a nursing facility. We can bill the client or a third party for the service.

Personal Care: Essential home care tailored to the client's specific needs. Recommended as a supplement to the care provided by friends and family.

  • These services assist with the daily activities of living.
  • Initial in-home consultation and development of care plan
  • Hours scheduled and structured by client/family/agency recommendations
  • Services provided may include: bathing, dressing, grooming, personal hygiene, mobility assistance, meal preparation, shopping, transportation, housekeeping, and laundry.

Premier Care: Most extensive care provided by our most experienced aides. Recommended for clients living alone or needing extra attention.

  • Impartial assessment and in-home evaluation of needs by RN

  • Monthly nurse visit / update sent to family
  • Medication Management/ RN oversight
  • Weight monitoring
  • Meal preparation/ Nutritional needs monitored by RN

  • Personal Care Services by our Star Team
  • Monitoring of scheduled visits with ClockWork

24-Hour Care: Carefully planned live-in care. An affordable service for clients who need assistance around the clock.

  • Two or three carefully selected home-care aides assigned on a permanent basis.
  • Scheduled in rotating 12-hour shifts, aides can fill in for one another.
  • Rotating shifts insure that the client-caregiver relationship remains professional.
  • Aides have time to attend to their own personal and family obligations.
  • Note: Aides must receive adequate privacy and rest when on duty.
  • Billed at low hourly rates.
  • Flexible weekly schedules, e.g. only work days or weekends, to meet client needs.

Bathing: Insures the well being and safety of those who need assistance with bathing. One or more visits per week as desired.

  • Full assist in and out of the shower/tub
  • Soaping, rinsing, and complete drying
  • Shampoo/shave
  • Lotion application for dry skin
  • Skin checks for breakdown
  • Fresh change of clothes
  • Hair care
  • Bathroom pickup

Hospital/Nursing Home Sitting: For the emotional support and attention needed by a convalescent.

  • For a loved one who is in the hospital or skilled facility
  • Star Team member serves as a patient advocate.
  • A kind word, a gentle touch, and a smiling face can distract patients from their worries.
  • Report back to family members.

ASAP! Service: For handling the emergencies that can arise when someone is alone at home or in a medical facility.

  • RN phone consultation with the client within 1 hour
  • Home/hospital visit within 2 hours
  • Direct "eyes and ears" evaluation of the situation
  • Report back to concerned family/friend with recommendations
